​Also used by the NHS to determine if an individual requires continuing care, our teams use the domains as a framework to assess the clinical needs of incoming residents. The domains cover a breadth of areas, including medication, mobility and cognition to create a comprehensive profile of a new resident’s requirements.

​Each domain is assessed non-intrusively and sensitively by a member of our team through an in-person meeting, enabling us to precisely tailor your care and create a seamless transition into our residences.

1. Breathing

​We carefully consider whether a person has conditions that affect their respiration, such as asthma, heart failure and pulmonary fibrosis. Our team carefully assesses if you are short of breath, the causes and the degree to which it affects your daily life, as well as your past responses to management, to identify the optimal level of support.

2. Nutrition

Our experts incorporate nutrition into your care plan with expertly prepared, balanced and flavourful menus from our in-house kitchen teams. During your assessment, they will discuss your dietary needs as well as any challenges you experience with eating, including choking risks or the need for a PEG tube.
We carefully tailor our menus to ensure the right nutritional balance for our residents and to help manage long-term conditions like diabetes and heart disease as part of our holistic approach to care and wellbeing.

All meals are prepared in-house by our talented kitchen teams who seamlessly adjust the menu to meet personal preferences, cultural and religious requirements, and to include our residents’ favourite dishes.

3. ​​Continence

This domain focuses on good bowel and bladder function. Our experts approach this topic with tact and sensitivity to identify the level of support that enables you to live in comfort and dignity. Solutions range from potential procedures and treatments to medication and installing additional fixtures in your suite.

​4. Skin Integrity

We will enquire about the condition of your skin, particularly any areas that you are experiencing issues with, such as wounds or areas experiencing pressure or breakdowns.

​Our approach considers a range of solutions, including preventative care measures, such as regular repositioning with the help of our nursing teams, equipment that relieves pressure on affected areas and medication.

​You can also receive tailored wound management after surgery or injury, mitigating the risk of infection and complications with 24-hour nursing oversight and specialist support.

5. Mobility

Our experts will carefully consider your range of motion to create personally tailored mobility solutions, including hoists, slide sheets and handrails alongside 24/7 support from our nursing teams. We also provide specialist support for individuals with long-term conditions like paralysis, hemiplegia and spasticity.

​Carefully considering mobility helps to mitigate the risk of fall and injury in residents, as well as maximising your independence by providing the right level of support to help you pursue your own routine and interests.

6. Communication

Our assessments include detailed reviews of an individual’s ability to express themselves, verbally and non-verbally. We use this information to create a personalised solution that helps an individual make themselves understood in our care, encompassing cognitive impairments, medical conditions and the use of communication aids.

7. Psychological and emotional needs

We respectfully and sensitively assess your psychological condition and emotional wellbeing. Our team considers your emotional state, any mental health conditions you have experienced or are prone to and past diagnoses to create a bespoke programme of treatment and support.

Where appropriate, professional mental health care and sensitive emotional support will be seamlessly woven into your care plan to help you adjust to life in full-time care and approach each day with confidence.

8. ​Cognition

This domain relates to an individual’s comprehension, memory and decision-making abilities. While we consider this domain for every incoming resident, we apply special focus for those with conditions like dementia, stroke or brain injury, as these can impair the ability to understand risk and cause disinhibition. We work closely with loved ones to identify a level of support that promotes safety and comfort while protecting that person’s independence and preserving their individuality.

9. ​Behaviour

We take the time to identify patterns of behaviour that may put you or others at risk during your care. Understanding these behaviours is essential to delivering care safely for you and our team. We proactively consider your requirements and history to create a soothing environment tailored to your emotional wellbeing.

10. ​Medication and therapies

All residents at Loveday receive a completely bespoke programme of treatments and medication, expertly delivered by our experienced nursing teams with 24-hour oversight. We coordinate with your loved ones, current carers and surgical team where relevant to ensure seamless continuity of support as you transition into our care. Our expert nursing teams ensure medication is delivered consistently, arrange treatments and provide enhanced monitoring to identify side effects and adapt your care as your needs evolve.

11. Altered States of Consciousness

We provide specialist support for residents who have conditions that cause them to lose consciousness, including epilepsy or stroke. Our team will carefully assess your history with the condition, the frequency of the episodes and any additional symptoms to identify the right level of enhanced monitoring, oversight and intervention required to promote safety, comfort and stability in our care.

The last domain captures additional needs that require consistent oversight and support, but are not covered by other domains.
